I would like to block out ambient noise when commuting/flying etc

    Wax ear plugs only muffle the sound a bit.

    Next step was "in-ear" Sennheisers but I must have small ear canals as even the smallest of the 3 rubber adapters supplied were too big to fit in my ear canals.

    Cutting out all ambient noise is impossible as your body will still transfer some vibration to your ears. Active noise cancellation removes even more noise and then add some audio you like and the outside world should disappear (just about).

    Active noise cancellation is not totally necessary - here are some In Ear headphones which are small at the nozzle (where the sound comes out) and should be a bit kinder on your ear canals. Comply tips would give even more comfort and noise blocking power.
